the following documents to The Office of the Registrar:-
A copy of marriage certificate

A copy of your social security card

Another form of major identification (ex: driver license or passport)

A copy of divorce decree
Send your documentation, clearly stating the changes that you
would like to make, to: The Office of the Registrar, Elmira
College, One Park Place, Elmira, New York 14901 or fax at (607)
